At ICare Resource Limited our reablement care service provides families with a tailored care at home service designed around your needs, enabling loved ones who have had an illness, accident or have just come home from hospital recover at home. Our reablement service is aimed to get you back on your feet and support you to regain and maintain independence.
Our reablement care service is a flexible care option enabling you to stay at home with support from a professional carer for a few hours a week through to multiple hours a day, or even sleepovers or waking nights.
Reablement care enables you to continue to live the life how you want to, on your terms, providing support with anything from basic domestic support, companionship, personal care, medication help, through to support with more complex conditions such as dementia or Parkinson’s.
CQC regulated home care
Unlike introduction agencies we are fully regulated by the Care Quality Commission (CQC) giving you the peace of mind that the care and support we provide is regularly monitored, inspected and regulated by the independent regulator of health and social care in England.

Locally managed reablement care
Our professional carers are matched to our clients to support them at home and we pride ourselves on delivering a highly personalised home care service underpinned by basic principles such as continuity of carer, introductions before care commences, minimum visits of 1 hour and a genuine focus on health and social well-being.
Our carers are fully employed by us and they undergo an extensive recruitment and selection process including interviews, reference checks and an Enhanced Criminal Record Check. They are also trained in all aspects of the care we deliver including specialist training for complex conditions.
Our reablement service provides short term care and support by working with adults in their own home to facilitate early hospital discharge, provide rehabilitation and prevent hospital admissions. We will arrange support in your home to help you regain confidence and ability in carrying out daily living tasks. The amount of help needed varies with each person and their circumstances. You might need only a few visits a week or several visits a day. Over a period of up to several weeks, the amount of help you need should reduce quickly, as your confidence and ability improves
Our care management team also undertakes regular monitoring of the care we provide, including an initial review after the first 4-weeks, formal reviews at least every 6 months, annual feedback questionnaires and ongoing monitoring of any concerns, events or issues. We also invest a huge amount of time in training, supervising, developing and supporting our carers which is key to enabling them to provide the highest quality care.
What is Reablement care?
Reablement care, also called intermediate care, is a type of short-term homecare that may be necessary after a hospital stay, accident or an illness to enable you to become independent again. Some people might also refer to reablement care as post-operative or post-surgical care, but these are slightly less specific terms as they don’t suggest an obvious end date, whereas reablement care is specifically aimed at people who will regain their independence after the period of care.
Reablement care usually runs for a maximum of six weeks, although the average is about two, and it consists of support in the home with tasks to increase independence, such as mobility support. An integrated care team will usually be involved with reablement care, meaning professionals from all areas of the NHS and the wider social care team may be involved. This can consist of homecare workers, nurses, occupational therapists, physiotherapists, social workers and doctors.

Companionship
Company and companionship from a carer matched to you so you can enjoy life with the right level of stimulation and emotional support.
Personal Care & Hygiene
Help with your personal needs such as dressing, personal cleanliness, lavatory needs and grooming, all with dignity and discretion.
Medication Help
Our carers are trained in supporting you with managing and administering your medications. They can collect prescriptions from the pharmacy.
Staying Active
We’re able to work closely with occupational therapists and other health professionals to ensure your mobility needs are met.
Shopping for Food & Essentials
We can help you with shopping whether it involves popping out to the shops or arranging an online delivery.
Light Housekeeping
This may include washing and ironing, cleaning rooms or vacuuming, to ensure your usual standards of cleanliness are maintained.
Meal Preparation & Nutrition
Your carer can help you plan and prepare nutritious meals to ensure your dietary needs are met, at a time that suits you.
Social Activities & Connecting
Your carer can support you to continue to socialise and keep close to family and friends.
Transportation & Errands
This may involve helping you get and about in your local community, going out on trips or running errands on your behalf.
Hobbies and interests
Continuing to enjoy hobbies and interests helps maintain your independence and our carers will support you to do this both inside and outside of your home.
Specialist Care
We will monitor your health and well-being outcomes and our carers are trained to support you with complex conditions such as dementia.
Reablement care FAQs
Reablement is a short term assessment and support service, lasting six weeks or less which helps you to do things for yourself rather than relying on others, such as washing and getting dressed. This will take place in your own home-setting.
- If you are 65+ and requiring a support package (ie a Domiciliary Care Package) or need an increase in your existing support package to help you with your daily living activities; and
- if you have experienced a crisis, such as illness, deterioration in health or sustained an injury.
You must be referred to the Reablement Service by a Health and Social Care Worker (eg General Practitioner, Nurse, Social Worker) either on discharge from hospital or from your local Health and Social Care office.
You will receive a visit from the Reablement Occupational Therapist who will carry out an assessment of your needs. This assessment will look at your daily living activities and will identify what you can and cannot do for yourself. From this a plan will be agreed with you. The Reablement carers will then work with you to practice these daily living activities to help you regain independence and confidence. As you improve the length and frequency of the visits will be reviewed and reduced by the Reablement Occupational Therapist.
Many people can live in their own homes independently after Reablement. However, if you still have ongoing need you will be referred, by the Health and Social Care Worker dealing with you, to other services.
The Reablement service is free of charge and is not means tested.
